It isn't common, but it has happened to me a few times now. Of course, most pumps just push on, so there is no rotational force to make it come undone.

It was a brand new tube, so maybe some aren't done up as tight as others? That doesn't help when you are standing on the side of the road without a very small spanner to tighten it with.

@ David tcr bugger also happened to me last week at the Grandslam a friend tightened the valve with some pliers seemed to do the trick.. 
I have a cannondale mini pump similar to the pocket rocket, light and effective.
A pump???? What is that, I thought most riders carried CO2 inflators nowadays???? LOL
a good set of lungs and a hose with a presta valve adapter?
